Licence Form of Particulars and Signatures

This is the licence agreement between the Licensed Institution and the Supplier, which both parties sign (the Licence Form is sometimes known as a Schedule 4), which comprises;

• the General Licence Terms and Conditions governing the use at the Institution of the products covered by the licence
• if appropriate and needed, a set of Special Licence Conditions which vary the General Licence Conditions (see above)
Agreement contact details must be entered on the Licence Form.
