SAS Software Agreement
Licence Information
Please note that if the required paperwork is completed, signed, scanned and emailed to Eduserv then paper copies do not need to be posted. Eduserv requires a Commitment Form, a Schedule 4 Licence Form, a Software Order Form, a Server Registration Form and a Purchase Order for the full licence fees plus £300 Eduserv admin fee per licence ordered made out to “Eduserv”. Scanned paperwork should be emailed to finance@eduserv.org.uk

Commitment Form
- agreement Commitment Form. One copy per participating Site should be completed and returned to Chest.

Schedule 4 Licence Form
- complete two copies and return to Chest to participate in the agreement. Two completed forms are required for each licence applied for.

Software Order Form
- complete and return to Chest, together with two Schedule 4 Licence Forms, a Purchase Order and Server Registration Form if required. One completed form is required for each licence applied for.

Server Registration Form
- use this form to register each Server that SAS Software is installed/used on. Complete and return to Chest. Server Registration Form - This form is required for the creation of the SETINITS. SAS need to know the manufacturer of the server etc. to enable the SETINITS to work correctly. If sites are concerned about the security of this then the forms can either be sent direct to SAS or communicated verbally to SAS.

Courses and Projects Declaration Form
- use this form to notify SAS of all courses and research projects in which the SAS Software is being used to a significant extent during the term of the Agreement. Note: this is on a voluntary basis only.

Student Use Agreement
- this is a document for your own internal use to record and ensure that students issued with a copy of the software have been made aware of their duties and obligations with regard to its use. The Chest/SAS Agreement requires that Educational Institutions licensing the software have an agreement in substantially this form with the students to whom they provide the software for home use only.

Discontinuance Form
- complete this form to remove products/platforms from your Site's licence. Note: this form should be sent to Chest before 1 May in any licence period to ensure that the invoice for the coming licence period commencing 1 August will reflect the change.

SETINIT Request Forms should be used to request SAS to supply a product authorisation code (SETINIT) for a prior release of SAS software, or for a product which is no longer available under the new Agreement. Forms are available on request by email to the Chest Helpdesk.

Documentation Order Forms are not available. Documentation is provided electronically in HTML format on the media containing the software. Copying of Documentation/Media is allowed.

Special Licence Conditions
- Read this document carefully to ensure that SAS software is being used at your Institution according to the terms of this agreement. The conditions in this document override those of the General Licence Conditions. In particular, please note the summary opposite.
