This is the list of sites (including affiliated Eduserv Chest sites) [for all software and data arrangements with suppliers] that defines the number of site licences or supported sites that may be involved in the agreement. Each site has a Chest site contact and a Chest data contact (these may be the same person).

Change of details form

This list may change from time to time as institutions are created, merged etc but any agreement with a supplier will be made with a dated copy of this list that can only be varied for that agreement with the mutual agreement of Chest and the supplier.
