• Commitment Form - one copy to be downloaded, completed and returned to Chest along with two Licences/Schedule 4s (see above) and a Purchase Order for the full fees made payable to "Eduserv".
• Modification Form - for currently licensed sites to add additional CPUs or change Technical Contact details.
