• Licence Schedule 4  - one copy to be completed, signed and returned
• Order Form  - one copy to be completed, signed and returned
• Additional Contacts Form  - one copy to be completed, signed and returned if additional pairs of Technical Contacts are required
• Change of Licence Request Form  - one copy to be completed, signed and returned when a Licensed Institution wishes to replace the "Designated System" on which its copy of AVS Software is currently licensed to run with a new "Designated System"
• Request for 75 User Option Floating Licence  - use this form if you have selected the 75 User Option and require a Floating Licence; the total Floating Licences plus Node Locked Licences must not exceed 75
• Request for 75 User Option Node Locked Licence  - use this form if you have selected the 75 User Option and require a Node Locked Licence; the total Floating Licences plus Node Locked Licenses must not exceed 75 - please complete one form for each Node Locked Licence required
• Request for Site Option Floating Licence  - use this form if you have selected the Site Licence Option and require a floating licence
• Request for Site Option Node Locked Licence  - use this form if you have selected the Site Licence Option and require a Node Locked Licence - please complete one form for each Node Locked Licence required
